If you have insurance that is not on this list, you can call member services to check your Out-of-Network benefits or our staff will gladly check on them for you.

REFERRALS
Under the New York State law, you do not need a referral for the first ten visits if all occur within 30 days. After that, however, you do need a referral from a doctor. Remember, too, that your health plan may require a referral for reimbursement, regardless of the number of visits. We can check the specific policy for you.
